SECTION 1 — FEDERAL GOVERNMENT PROGRAMS
Health & Home Care
Ontario Health atHome (OHaH)
Free government-funded home care for eligible Ontarians — including PSW services, nursing, physiotherapy, and occupational therapy. If your loved one has been assessed and approved, they may qualify for funded hours delivered by an agency of their choice.

Formerly known as Home and Community Care Support Services (HCCSS)
Self-Directed Care (SDC) / Family-Managed Home Care (FMHC)
A program under Ontario Health atHome that allows eligible clients and families to choose and manage their own caregiver — including through agencies like ProvideUcare. Particularly relevant for clients with language barriers, complex needs, or extraordinary circumstances.

Income & Financial Support
Old Age Security (OAS)
A monthly payment available to most Canadians aged 65 and older — regardless of work history. One of the most important and most missed benefits for newcomer seniors.

Guaranteed Income Supplement (GIS)
An additional monthly tax-free benefit for low-income seniors who receive OAS. Many eligible seniors do not apply — families should check eligibility every year.

Canada Pension Plan (CPP)
A monthly taxable benefit based on how long a person worked and contributed to CPP. Also includes disability benefits, children's benefits, and survivor's pension.

Canada Disability Benefit (CDB)
A federal benefit for working-age Canadians with disabilities. Ontario has confirmed that CDB income will not reduce ODSP payments — eligible residents can receive both.

Veterans Affairs Canada (VAC)
Home care funding, personal support, and health benefits for eligible Canadian veterans and their families. Can fund caregiver services through approved agencies.

Disability & Social Support
Ontario Disability Support Program (ODSP)
Monthly income and employment support for Ontario residents living with a physical or mental health disability expected to last one year or longer. Covers basic living expenses, drug benefits, dental, and vision care for eligible clients and families.

Ontario Works (OW)
Financial and employment assistance for people in financial need while they look for work. Can be a pathway to ODSP for clients whose disability is newly diagnosed.

Assistive Devices Program (ADP)
Government funding for assistive devices including wheelchairs, walkers, communication devices, hearing aids, and orthotics. Requires a referral from a health professional.

Developmental Services Ontario (DSO)
The point of entry for adults with developmental disabilities seeking services and supports funded by the Ontario government — including residential support and day programs.

211 Toronto — Community Resource Helpline
The most important number for any family navigating Toronto's social services. Call 211 to be connected to any community service, government program, housing support, food bank, mental health resource, or senior service in Toronto. Available 24/7 in multiple languages.

Connects seniors to phone call services, friendly visitor programs, and community supports across Toronto. Also provides information about services that help seniors continue living at home.

Home & Financial Support
Toronto Homemakers and Nurses Services
City-funded support for Toronto residents who need help with household activities of daily living and have limited financial resources. Fees are geared to income — some clients pay nothing.

Property Tax Relief for Low-Income Seniors
Eligible low-income seniors in Toronto can apply for property tax increase cancellation, deferral programs, water rebates, and solid waste rebates.

Somali Canadian Association (SCA)
A non-profit founded in 1987 serving the Somali community in Toronto. Provides culturally and linguistically appropriate services including senior programs, workshops on chronic disease prevention, health awareness, and referrals to government programs. Serves North Etobicoke.

Somali Immigrant Aid Organization (SIAO)
Provides culturally appropriate family counselling, seniors programs including nutrition, health awareness, workshops, and recreation. Also offers settlement services and language support.

ACCES Employment
Employment and settlement services for newcomers and immigrants across Toronto. Provides language-specific job support and connections to government programs. Serves communities including Ethiopian, Eritrean, Somali, and Arabic-speaking newcomers.
